I'm looking for a brief, practical measure which would assess global functional well-being in children between ages 4-12 for patients treated in a variety of inpatient and outpatient psychiatric programs. I have reviewed numerous measures which are too long, cost money or are not practical for repeated assessments (for example, pre, post and at follow-up points). I thought the answer might lie in Jeanne Landgraf's and John Ware's Child Health Questionnaire but they have not yet released the questionnaire to the public. Since I'm under considerable time pressure, I wonder if anyone out there has ideas? Or, if there were a minimum data set of variables to include for this population what might they be?
