Thanks for the response. If you have been using the questionnaire I'm interested in hearing more about it, if you have the time and inclination. (For example, in what setting, who adminsters the questionnaire, what gets done with the data). In my setting, we routinely collect data for the purpose of outcome assessment, but we face substantial constraints in terms of money (to pay for copyrighted questionnaires) and time (to administer long questionnaires).
