What actually happens in the brain during spontaneous recovery? How can we increase the probability of this occurring in individuals suffering from PTSD?

Why does exposure work for some individuals and not for others? Why does it make some people worse? Why do some people who got worse from exposure benefit from EMDR?

We are a long way from truly understanding any of these mechanisms including those that purportedly account for the effects of exposure.
