Spontaneous recovery certainly occurs especially for minor conditions. For example, if a person has had an okay childhood and then gets in a car accident, they may have nightmares for a few weeks which then resolve. It is like an infection that heals itself with time.

If someone had a horrid childhood filled with trauma, and then gets in a car accident, we see that those nightmares and many other longstanding symptoms don't spontaneously resolve. Treatment is needed.
