contraindications

Could you please tell me what all the known contraindications for the use of EMDR are in terms of the following three things:

(1) what the client brings to the table;
(2) the therapist's strengths, weaknesses, and qualifications; and
(3) the length/strength of the therapeutic working relationship?

I'd look carefully through all the posts here or read Shapiro's book to try to get the answers, but attempting to do so right now makes me feel even more anxious than I am already.
