Self-report Inventory for Disorders of Extreme Stress, Bessel van der Kolk, is an 11 page assessment of the clients subjective levels of stress applied to many levels of the 'self' (my words not this) such as: 1.) Alterationin Regulation Of Affect and Impulses. I believe this form is used ONLY by the staff at the Trauma Center in Boston Ma. But I may be wrong. The report is self administreted , either on papaer or via computur and then socred, giving the therapist and client a measurable view the clients level of distress over the past month.
