Although I am not sure I completely understand what you mean by situations that 'cannot be done or repeated'
I have an idea that may be helpful.
Apparently you have used exposure in vivo ( real-life exposure). What about using ' exposure in vitro' ( i.e. having your client imagine situations that happened but cannot be repated or situations that cannot be created at will) It is important for you to verify that when your client is imagining supposedly anxiety-engendering situations or thoughts he really does feel some anxiety. Only when he FEELS the anxiety he will be able to learn that the anxiety will go down even if he does not do his compulsions ( behavioral or mental).
