information online therapy

Hello!

We (4 students) do a research (study: social care in Zwolle/Holland) to online therapy via internet.
Questions we have:
-Which country has started online therapy and is now leading the world/ Which countries are leading Europe at this moment?
-Are there any diferences compared to Holland and what kind of? (for example: webcams, insurrance-payments, using of microfones, the kosts)
-How are the results in countries like Germany, USA, England?
-Can someone from Holland use a therapy-site in, for example, USA?

Can someone help us? Do you know any links? Thank you very much!!!
