The others did a good job of expalining DAP and SOAP. You asked for an example. We use SOAP at the agency where I intern so I'm more familiar with it. A sample note would read something like.... am using ALL CAPS to denote the steps "Client's facial expression and affect were sad(SUBJECTIVE) Client stated s/he is sad about death of loved one.(OBJECTIVE) Client is exhibiting early stages of normal grief.(ASSESSMENT) Referred client to local grief support group. (PLAN) Hope this was helpful.
