Re:DAP/SOAP NOTES

Hello,

You will find more info on search engine results if you put in "soap notes" as a search term.

This acronym stands for:

S = Subjective data

O = Objective data

A = Assessment

P = Plan

The DAP acronym combines the S and O into one Category, Data, without making a distinction between the two.

You may also see references to SOAPIE notes--the I = Interventions and E = Evaluation (of the effectiveness of the interventions).

There are other formats for client progress notes as well. Whichever systematic format you choose, be consistent in its use.
