    D = DATA
    A = ASSESSMENT
    P = PLAN

    Your best bet is to look at some charts to see how DAP notes are used in the real world.

    The DATA is what you see. The ASSESSMENT is, of course, your assessment of the data.

    The PLAN is what you plan to do.
