Regarding getting "stuck" on a SUDS of 2 or 3: There are many possible reasons why this might happen. 1) For some it is a "blocking belief" such as about fear of healing. 2) For some it is a loyalty to some cause or person (protecting a parent for example). 3) For some it is a matter of no longer suffering enough on that subject to warrant a return to it. 4) For some its the therapist that isn't being careful to explore the issues and stuckness. 5) For some it is a "part of the self" that is concerned about losing power, or "being done away with," as if there is an ego-investment in continuuing unchanged, or resisting change. Regardinghow to address a "part of self" that would rather not heal or be "released" from the past: An ego state apporach (which may or may not apply in your case or in any given case, but these are general thoughts) would be to invite whatever stands in the way of the SUD going down further (or the VOC going up further) to come into the "conference room in the mind's eye" so that its concerns can become known. Voila! Usually the issue or part is evident and visible and can be directly interviewed (with the front part of the self reporting what s/he hears and sees from the part in the conference room in response to the therapist's questions). It's often: a parent, and the attachment is being protected; a perpetrator, whose secrets are being kept under threat; a child part, that feels weak and not strong enough. Ego state work can help orient that part to present time (the perp was 30 years ago, now there are positive resources, the adult self will protect, etc).
