I hope your colleague finds the questions useful. I certainly don't want anyone getting in trouble with the licensing board but if your colleague would like any additional input, it might be worth considering whether answering the questions would really disclose any potentially identifying information. One possibility would be to get the client's consent before answering the questions. It seems to me that the licensing board would have a hard time objecting if you are seeking information on the behalf of the client with the client's consent.
