Dysfunctional Attitudes Scale

I have got hold of a copy of the DAS and would like to use it with my clients, however, I do not know how to score it. The original reference is one I can't get hold of. I have Form A and from the literature understand that it has 2 factors. If anyone can help me with scoring it I would be grateful. Or if anyone has any references with the scoring I woudl be grateful to know them also.
