Colleagues recommend: Raphael Melmed's Mind, Body and Medicine: An Integrative Text published by Oxford Press 2001. It covers a number of medical conditions, but has a chapter on Fibromyalgia and several chapters on CBT approaches as they are applied in behavioral medicine. New Harbinger (www.newharbinger.com ) has 2 resources: Starlanyl, D. J. & Copeland, M. E. (2001) Fibromyalgia and chronic myofascial pain: A survival manual (2d ed.), which is aimed at patients and largely CBT (including relevant skills and information), and Starlanyl, D. J. (1996). Chronic myofascial pain syndrome: Guide to the trigger points, which is a video demonstration of "self-care physical therapy techniques".
