Treatment for personality disorders in children and adolescents

I'm a graduate student and neophyte cognitive-behavioral therapist and have had a tremendous amount of trouble finding treatments for personality disorders in children generally and narcissistic PD specifically. Could someone please send me a reference? My suspicion is that not much has been written about CT or CBT for these disorders. If so, then what types of treatment approaches have people been using?
