Personal Disclosure

I work at a facility for mentally ill and we have come across the problem of how, much as a staff we should, or should not disclose to our clients that we work with. For example if we are married has children were we live and others things about our self. I was wondering if any one out there might have a disclosure policy at their facility that they would be willing to share so we might get an idea of how to put one together for our program. Just so that you will have a little more insight our program is a residential facility for mentally ill 18 years and older.
