I think it really depends on a number of factors. First of all, those practitioners who have developed a certain notoriety in a given specialty will see more patients who fit within that specialty. As an example, one psychiatrist here in Atlanta has developed quite a name for himself in the treatment of sexual offenders, so he does nothing else. Most of the practitioners I've worked with, though, tend to limit themselves only in terms of age group -- some refuse to see children, for example -- or mental health vs. substance abuse. This is not universal, I'm sure, but it's what I've seen. Personally, I work only with adult clients (who have primarily mental health problems).
