Writing Effective Treatment Plans

I have been chosen by the community mental health agency in which I work to give a presentation on Treatment Planning for the 40 clinicians in the agency. They want me to focus on constructing "measurable" goals that managed care will like and approve. They also want me to give some "buzzwords" for managed care as well as tie the diagnosis to the treatment plan, etc. If any of you have any ideas or suggestions that you might be able to pass on to me, it would be much appreciated.
