    Many years ago, I attended a workshop given by Jack Rosberg, who developed an approach called Direct Confrontation. Check http://www.schizophrenia-help.com/may.htm and http://www.schizophreniatherapy.com/program/history.htm .

    An Adlerian variation would be to enter the delusional system as an equal, or slighly below, to establish contact and then question the beliefs very gradually. The Socratic method might work, exploring the consequences of beliefs and actions.

    Also look at the book "Essays in Schizophrenia," by Bernard Shulman, who is an Adlerian psychiatrist.
