    I am an RN working in an adult psychiatric facility. I have been seeking information on model programs for the chronic psychiatric patient. I have begun to implement some changes on my unit, educate the staff, (particularly regarding schizophrenia), and document changes. My goal is to change the focus of client care to 1)individual, goal oriented; 2)discharge focused from day l of entry into the hospital; 3)an increasingly 'realistic' environment, giving the patient more control over his life. In addition, I strongly feel that patients should be discharged with follow up from someone they trust until another trusting relationship can be established in the community they are discharged to. I work with a team who are open and willing to try new things.

    This population has been underserved for years. Current methods of practice from a nursing point of view do not incorporate an environment conducive to success in the discharge for my patients. Although the idea of PACTS teams is wonderful, these services are not yet available in our area.

    I found your "page" looking for information on change theory. I have big goals for a small piece of my pie. But any small step to bring this neglected population closer to some kind of self determined happiness will be success in my eyes. Any information you can share would be appreciated.
