Looking for info on brain injury outcome evaluation measures

Our Brain Rehab team is reviewing outcome evaluation measures for our community reintegration program (outpatient). Our client population ranges from severely brain injured clients, to highly functioning clients who have had minor brain injuries. I am looking for specific information on how to get copies of the following tools: 1) Portland Adaptability Inventory; 2) Functional Assessment Measure; 3) Global Assessment Scale. Can anyone tell me how to obtain these scales and instructions for use? I have already reviewed research articles describing reliability and validity. I am also interested in hearing about any other outcome evaluation measures people can suggest. Our major concern is that the tool be sensitive enough to apply to people with minor brain injuries, as well as the severely impaired clients. Thank you!
