Outcomes Projects: how do YOU do it?

With the recent emphasis on outcomes assessment within our field, I wonder if people in this forum are willing to share experiences about: how clinical outcomes projects began in your settings? what role (if any) do staff and clients play in the design and development of your outcomes work? how - and if - you involve families? how (knowledge dissemination) and for what purpose (knowledge utilization) outcomes information is used in your setting?

final question: Has anyone developed a good family satisfaction/service feedback survey?

I realize this is a broad set of questions, but I would love to hear experiences in all or some of the above, especially if you have had an interesting "Aha" in some particular area.
