Rensis Likert was the Director of the Institute for Social Research at the University of Michigan. It is still a thriving operation and I would suggest that you write or preferably call the office of the director there and inquire how you can obtain a copy of this instrument. His book, New Patterns of Management is one of the original writings developing his approach to management which he called system 4.
