Info. request: Outcomes assessment of Anxiety Disorders

I work at an adult outpatient mental health clinic (I am a psychologist). I have been asked by my boss (a psychiatrist) to find brief user-friendly outcomes measures for anxiety disorders. I need one quick measure for each DSM IV anxiety disorder. I would prefer paper and pencil instruments that could each be administered in about 5 minutes to be used at baseline and on a weekly basis throughout treatment. Do such measures exist?
