Iam aware of The issue of presence of Schneiderian first rank symptoms(SFRS) in DID.I have Carefully excluded that possibility.In fact client is on clozapine and BPRS has fallen from 65 to 31 In about10 weeks.Iexpect further improvement in the ensuing 6 months.Now client has dissociative experiences in which she comes out of her body mostly in the wake of stressors and infact this was not evident when she was psychotic .Any takers of the theory of schizophrenia as a defense(I probably read it in a textbook of psychiatry bySilvanio Arieti,1961)
I must add some clinicians are quick to diagnose DID in patients with sexual abuse thereby deying them a very valid treatment at its earliest stage just as some rush into diagnosing schizophrenia with the presence of SFRS.
