Derealization is a common symptom that occurs with some anxiety disorders as well as with dissociation. It means that the person feels as if things around them aren't quite real. It is similar, but not identical, to depersonalization, in which the person feels as if s/he is not quite a real person. We always urge ALL clinicians to screen all clients for a dissociative disorder so that when they start EMDR, the processing can go smoothly. Derealization itself is not a reason not to proceed with EMDR, but a full fledged dissociative disorder requires a special protocol so that symptoms like derealization are not overwhelming.
