Re-reading my reply I think I may not have gone far enough in my observation as your patient keeps returning to her 'self image' as being what she thinks of as 'self esteem.' What I intended to say, which I may have already indicated is that an activities approach may help. Doing things, small skills perhaps so that she may be able to externalise herself little by little & move away from being continually concerned about her self image. Group activities such as playing board games, play reading, walking in a natural environment & learning about the habits of other creatures, swimming lessons & so on.
