Ego-dystonic, negatively reinforced, unwanted, anxiety-producing sexual obsessions (i.e., ocd) would be treated quite differently from ego-syntonic, positively reinforced, pleasurable sexual fantasies. For example, exposure would be a central component of treatment for OCD while abstinence might be recommended for sexual addiction. What are the client's goals?
