Norm Epstein and Don Baucom's new book - Enhanced Cognitive-Behavioral Therapy for Couples (American Psychological Association, 2002) includes a chapter on assessment which discusses a number of questionnaires which may be relevant. However, they're focusing on couples therapy in general, not specifically on family therapy for families in which there is a drug abuser.
