I haven't had time to look up any references but I'm pretty sure that we discuss this in The Clinical Practice of Cognitive Therapy (Freeman, Pretzer, Fleming & Simon, 1990). I'd also check Judy Beck's book, Cognitive Therapy: Basics and Beyond, and Chris Padesky's article on schema change which appeared in Clinical Psychology and Psychotherapy (In 1990, I think).
