Conceptual Basis of Cognative Behavioral Therapy

Hello,
I came across your website and thought I might ask for your help in retrieving information on CBT. I am not a psychologist but have a strong background in the sciences. I am hoping you could recommend a book or review article on the conceptual basis of CBT and chronic illness. I really want a text that looks at the overall concepts with examples or case studies relating to individuals with chronic illnesses - pain, fatigue etc.
