By the way, if anyone is interested in the complete Control-Mastery formulation I wrote for this case after the treatment was completed, see "A Control-Mastery Case Formulation of a Successful Treatment for Major Depression" which was published in the new journal "In Session: Psychotherapy in Practice, Vol. 1, No. 2, pp.3-17 (1995)." In that paper I explain in detail how Kathy's pathogenic beliefs developed, following her history through the two major depressions and up until she comes to treatment. Due to copyright laws I was unable to duplicate much from that paper in this forum.
