Re: DAP format for progress notes

it's my understanding, DAP format which is Data, Assessment, Plan incorporates subjective statements and observed behaviors in a brief narrative summary. an assessment is then made based on data collected & recorded as well as a plan.
fairly simple, without benefit of an intense training or inservice.
