I agree with Monica, but Alex sounds like someone who is very tired of having to deal with beaurocratic BS and redtape just to try to get his patients taken care of. But it appears that you both have overshot the runway. I think the ethical dilemma is also having to make a choice like this for your patient at all. Although it is difficult, I would have to present the patient with the facts and let him/her decide. If they are not capable of making the decision, then I would imagine that would have been picked up on earlier. AND, why are we doing your homework for you anyway?
